Your ID, insurance card if you have one, and a list of any medications. Arrive 10 minutes early so paperwork doesn't eat your appointment.

Brightwater is in-network with most major PPO plans and happily files claims for the rest. Not seeing yours? Tell us your plan when you book and we'll verify your benefits before you arrive — no guessing at the front desk.
